A, In a shoulder with no rotator cuff tendons there are few restraints to anterosuperior subluxation of the humeral head when patient attempts to raise the arm. Pull of the deltoid muscle worsens this by pulling superiorly and medially (arrow). B, With reverse arthroplasty, deltoid muscle lever arm is restored, providing a fulcrum around which the deltoid can pull to restore forward elevation (arrow).
