The posterior cruciate ligament (PCL) is a more medial structure and may be involved in coronal plane deformity in osteoarthritic varus knee. After release of medial structures, PCL may still be affecting medial gap, especially in flexion, and may need to be released to equalize gaps. This can be done intrasubstance or off tibial insertion with or without piece of bone to effectively elongate ligament. PCL less likely acts as central tether in valgus deformity because of its more medial location. If it is involved in more severe valgus deformities, it may need to be released. SEE TECHNIQUE 7.5.
